You can start with a regular bubble. Hide the bubble layer, you won't be using it until later.

Now, here is the hard part. You need to make some fake tits and leave the area in the crotch naked. It doesn't need to be perfect. You can use naked pics of other girls, just make sure the skin tone is about the same.

copy the layer with fake tits/crotch. Blurry that copied layer, then use the bubble layer to select and delete parts of the blurry layer.

Add some effects to the blurry layer and you will have this final product.
