Almost never happens when and where it's needed most. Mods aren't around/have no standards, and the janitors might as well not exist because most of them have the same eyes and minds as the shitposters. The majority of the userbase is impossibly stupid, lazy, and immature, and don't report anything that isn't exceedingly obvious like CP. The small things matter a lot. You ignore them, and it sets a terrible precedent that starts to make the impressionable and non-critically thinking users think something is something it is not.
