This is what happened.

A hospital in the general area was taken over by Taliban. The commander of Afghan forces called US Special Forces Command and asked them to level the the hospital. HOWEVER. He forgot to mention that there was more than one hospital in the general area. So when the AC-130 went in, they targeted the first hospital that they could find, unaware that there was more than one hospital in the area.
