Being 95+% effective means something is eventually going to get through. In all our defense simulations it wasn't a failure in the missile defense system, it was the fact we run out of missiles. The Russians or the Chinese would overwhelm defenses with a massive number of warheads mixed in with non-nuke dummy warheads. Something eventually rolls a 20 and gets through by sheer numbers, or we just run out of ammo. The systems were never intended to stop a full-blown MAD end-of-the-world scenario where all nuclear powers launch a thousand nukes at each other. They just provide a bit of extra damage control for critical targets while we watch the rest of the world glow in the dark. They're intended to stop rogue countries with a limited nuclear arsenal.
