They want your info so you need an Organization or a trust or an NGO or non profit, unless you want your personal details.

You also need a mailing address and contact point.

I would suggest a PO box and burner number.

So the cost to register a non profit (500 bucks I would guess) an agent for service (probably 200 dollars)

A PO box and phone number is guess around a cool 1k to do this legit.

Or you could try and make it up by lying on a federal form which I believe is a felony.
