If everyone in your country is so poor that the government had to start paying for universal healthcare for all citizens because no one could afford their own healthcare, your country is not a first-world nation. If most people in your country can't even afford to purchase a car, you're not a first-world nation. If your country has never even been to the moon, you're not a first-world country.
