Anti-vaccine does not limit births in the way you think.
High mortality rates cause people to breed more- lowering mortality (from disease) can decrease births.
But the decrease from lower mortality is offset (more births) by giving resources to these people.
The solution is to provide health services, but not food/shelter services.
