its real. it happens when the chromosomes dont split properly during the meiosis. there is also XXX. but both (as far as i know) are sterile and unable to reproduce. both are mutations that nature didnt intended to exists though.
theoretically there should be just x and just y around, but there is chance those wouldnt be able to live and die before birth (would have look it up.
as for XXXY, well for that to happen both parent would have to fuck up the meiosis for this to happen, no idea if it would be able to live
