Why is healthcare so expensive in the united states of america vs the rest of the world?

actually I know the reasons
>US Health Insurance companies make all of their money by scamming healthy people that are too ignorant to understand their own needs and finances.
>Doctors/Hospital inflate their costs when they bill to Health Insurance companies because they know they can afford it due to all the scamming.
>Medical product manufacturers inflate their prices because they know doctors can pad it into their bills to the insurance companies.
>Health Insurance companies increase premiums because their cost of business goes up.
and this cycle never ends... obviously private education system plays a big role in this cycle as well, but that's another thread.

Is there a way to provide healthcare in the United States without the need for middle-men shaving BILLIONS off the top specifically for-profit?
