record it to vhs, record it off of the vhs, make a screenshot, use some kind of data corruptor tool or fuck with it using audacity with raw data
The first part is recreating the VHS effect. You're better off recording the image on actual videotape and getting it that way. Otherwise, in Photoshop, try shifting channels, play with brightness/contrast, posterize, etc.
There is a font like that that is actually available for free. Google "VCR font" or something like that.
Bottom part is motion blur.

just fuck with the color layers to simulate chromatic aberration and then add non-monochromatic noise while fucking with the curves
Filter > Other > Offset
and tick 'repeat edge pixels'
