Also, I once had a low grade sinus infection for several months, it made me fatigued and sick all the time and gave me swollen lymph nodes and bad breath, but because of some concurrent issues that my doctor blamed my symptoms on, it wasn't noticed until I had a dental X-ray and my sinus showed up cloudy. Those symptoms could be from something you haven't even considered.

idk what im talking about of course but your symptoms kinda remind me of a story I heard recently of someone who had babesiosis. Its transmitted by ticks like lyme disease and CDC gives a loose list of symptoms but the person infected recounted severe fatigue and tiredness, brain fog, and joint pain. They had to drop out of college because of their tirednesss and constant faigue and they were contemplating suicide when she finally got diagnosed. You should do research or ask about it because its pretty uncommon and not most family doctors would think of it. Idk what tests you went through but you can check around the CDC website and see what tests they do to look for it and what youve got tested for so far.

My grandmother got diagnosed with early onset arthritis at 25, turned out she had scleroderma, died a shitty death. Doctors are trained to look for common issues, it's clearly either an infection, allergies, or cancer if lymph nodes are involved. Do your part to check off obvious causes and go to a different doctor.

Make them do a full fucking lab workup, everything from your cholesterol to your testosterone. Get a copy of your lab results and take them to an endocrinologist or even just another primary care physician for a 2nd opinion.

If you have good insurance you can go directly to any doctor you choose. If you have bad insurance you go to a family doctor who decides if you can go to a specialist and be treated (after getting more permissions from the insurance company). The family doctors job is no longer to heal you but to act as a gatekeeper to the medical system, like death panel lite and deny you service.

Good and bad insurance is relative now though, because bad insurances (Medicaid for example) is often 100% free while good insurance has high deductible and co pay so it's too expensive to use.
