Engine shakes badly when car is started warm, e.g.

Totally irrelevant. I had 3 different cars do that.
You are going to ask me what car if I tell you "My car pulls to the right"? No, you will tell me to check tires, wheel balance, tie rods, and then control arms and brakes if all else fails.
Same exact thing here - it's a general issue. You can probably guess I am not running a carb, and that's about all that's relevant.

Could be a misfire in one of the cylinders, wither from bad coil pack, bad plug, bad injector,bad injector pigtail.
Even though a misfire is pretty noticeable it docent always trigger a CEL

obviously one or more cylinders is not firing
it could be the fuel rail not being pressurised properly or ignition/electircal, those are the most likely causes
then i would look into timing, crank sensor and such

I don't understand how any of these are related to operating temperature though, especially in the sense that it runs perfectly when hot and cold, but it's mid-range that's the problem, and only when STARTED while partially cooled down.
It's np to change fuel-rail, plugs, fuel pump, and other basics, but that's kind of guessing.

Temp can mess up the fuel delivery system. For example had a ranger that would not start in the summer after driving for a little, shutting down, then restarting. Replaced fuel pump but wasn't the problem. Turns out the rail was bad, cause in warm weather, rail would heat up so hot, when the car was turned off, fuel in the rail would vaporize, causing vapor lock. But after it cooled down and re-liquified, truck could start.
Sorry this is a off topic story but just showing how temp could affect fuel delivery.

Hey OP, not sure if this will help st all, but what fuel do you use?
I know for sports vehicles/luxury vehicles they need the more expensive fuel.
When my friends put cheap fuel in their old BMW the thing rattles like a mofo, I told him to switch to expensive fuel and it solved his issue.
