What did they mean by this? Do they add coffee and cream to everything they add sugar to? Do they keep sugar along with instant coffee and creamer powder all mixed together and just mix it together like slop? I'm confused.

Say you're ordering a coffee. "Medium with two cream, two sugar." The majority of that is coffee while two cups of cream and 2tsp of sugar are added. Now imagine a medium cup of sugar with some coffee and cream sprinkled in there.

try it at home fill a cup about 3/4ths of the way with with sugar then put the sugar in a pan mix equal parts sugar and water and cook until clear and thin, you now have liquid sugar or simple syrup. take your cup fill it 3/4ths of the way with simple syrup then make a very small amount of coffee and put it in then proceed to add creamer, stir, and drink (warning it wont taste very good but it is drinkable)
