First world/second world/third world all refer to the different alliances during the cold war. "Third world" has just picked up the added meaning of poverty along the way as the third world were unaffiliated countries who were poorer and so had no investment in a nuclear arms race.

That said, there were still plenty of poorer countries who were allied with the west mainly out of a need for protection that are considered "first world" due to their affiliations in this period.
