Having said that I own the album on vinyl and CD, the CD comes with a large fold out booklet that even when folded back up the M and E get cut off, it does seem intentional? but also maybe not? Hard to say. the fold out has a large drawing on it so it's possible that when making the schematics or whatever for the CD a mistake was made so it appears that way. The full words can be seen when the booklet is folded out

At the end of Samurai Sword he's mauled by the bear that is his girlfriend, and in My Warm Blood he bleeds out. He does greet the reaper (the black ship) in The Sun though.

Samurai Sword is inspired by The Sword of No Sword, a book I've read but I'm unsure how Phil alludes to it in the song. The Glow Pt. 2 is a lot less conceptual than you're painting it to be I think. Mount Eerie is definitely a concept album, but the black ship has never struck me as being a grim reaper reference. Phil explains in Headwaters (the extensive explanation of Mount Eerie) that the ship is supposed to symbolize fear or the feeling of having to run away from everything.

It's a song that Phil released under The Microphones as a single, along with the b-side Antlers on a 45" record (that I own!). It is more widely distributed as a track on The Microphones singles/rarities compilation called Song Islands. An alternate version of the song can be found on The Glow Pt. 2 extras CD that came out in 2008 I think.

Phil played this song live a lot throughout the 2000s, it can be heard on many bootlegs. This is a really cool faster recording of the song, nice find.

It's possible that there are some bootlegs out there on the Mount Eerie Preservation website. A whole (out of date) forum for Phil's music that has a bunch of mediafire links, some of which are dead.

Also here's this really good recording of a show where Phil played a whole set of Thanksgiving (Adrian Orange) songs and his own song Thanksgiving. Phil's performances of those great songs are really cool, I recommend listening to Thanksgiving if you haven't already. Phil has gone on record saying that Adrian is one of his favorite song writers if not his favorite period.

it was the first Phil album I ever bought weirdly, I was probably 14 and I just saw it in a record store and bought it. I didn't really get into Phil's music until I was 16 though, that's when I dove into everything that he had out at that time. Now I'm a huge supporter and collector of his music.

I love Live In Japan, it's fun hearing Kyle and Calvin sing in the background and play melodica badly.
