The approximation of the overtone series precludes the invention of equal temperament systems like the 12 tone one we use now, so you're pretty hard pressed to define any music at all that fits your rules, that is if you define sound collage, random shouting and pure noise as melodic (which it's not).

A lot of metal is not conforming. First of all you've got a lot of chromatic chugging in death metal. But even take a more melodic riff like freezing moon by mayhem:
It starts with an arpeggiated Em triad, then the chords are Bb5, A5, G5, F5. It sounds generally kinda phrygian/locrian minor-y but you can't place all those notes in one tonal center and the melody doesn't obey standard key change rules.
