Try just keeping a diary. Just consistently doing it should help, you'll also spend more time thinking about how to describe things when you're out and about.

Practice writing descriptions about real things, and people, especially people's faces. Then learn to critique with an emphasis on word choices. Start reading writers with an objective style and learn to pare your sentences to the essentials before moving on to other styles. Simply noticing how "empty" a word or phrase is, especially cliches, and then choosing to speak plainly is a good start.

Since you read Portuguese, I strongly suggest you track down and study the books by Antoine Albalat, e.g. A Arte de Escrever em 20 Lições. Perhaps there are others available in Portuguese translation too. His writing guides are among the best of the best.
