How do I do it? How can I train myself to just sit down for an afternoon and read for hours upon hours at a time?

Pomodoro technique
Identify a few genres that you like, then pick a few authors from each genre. Read a few chapters into each one and decide which authors envelope you the best into the book, making you feel that you have become part of the story. That is usually the best way to just sit down and read.
My grandfather, father, and I are all readers, but the eldest likes mystery and my father likes sci-fi. I thought I would enjoy sci-fi until Asimov killed it for me with his overwhelming amount of nerdiness. But there are still some sci-fi authors I enjoy, like Philip K. Dick, Robert Heinlein, and William Gibson's trippy af "Neuromancer".Ended up preferring the fiction books that involves mental introspection through the characters own habits and decisions.

>read a book undisturbed for more than an hour
Why would you want to?
Reading is hard. It's a good habit to get into, namely for the great benefits you receive for stimulating the Prefrontal Cortex.
Reading is like meditating. You focus on your object of study and let nothing distract you from studying that object for a certain amount of time.
Do you sit for hours on 4chan? Do it like that but with a book instead. This will probably work, unless you aren't actually interested in what you are reading.

By taking breaks and taking notes. Taking notes will engage you more in the text and you'll become more interested in what you're reading.
