Depends on what you're trying to do with the characters. Do you want to redeem him? Punish him? Use him as a vehicle for your critique of US drug policy in the post-Reagan era? If you know where the goalpost is, it'll be easy to map your route.

Maybe make it an act of God, like he crashed his car in a snowstorm and gets a brain injury or some shit. That way it symbolizes addiction can be out of the addicts control
