I think the mental illness is less about the outward and more about inner. Being trans seems to be an extreme manifestation of depression and escapism from one's current place in life. When someone becomes consumed with thinking their life would be better as essentially a completely new person this leads to crippling dysphoria. The worst part is more times than not its much deeper than simply chopping your dick off, leading to the high suicide rate.
