How do trans women who don't care about "passing" reconcile that with the fact that cis women are extremely appearance-conscious?
They're not as some sort of universal rule obsessed like you imagine them to be.
Only those who have been influenced by straight men and beauty product corporations enough really put an unhealthy amount of effort into it.
In fact, most people are pretty chill about it and put in a medium to minimal amount of effort.
I think you'll find outside of your sheltered circle, things are much different, and people mostly care about appearance only if you're shockingly above or below human levels.

The point isn't that they conform to anything, it's that they are very aware of how they look, and have a more intuitive sense of what looks good and what doesn't.

Cis women don't need to put much "effort" because with 10+ years of practice it becomes somewhat effortless, doesn't mean they're not appearance-aware.

The point is nobody normal cares that much about how they look that they'll inconvenience themselves to look really good instead of average unless there's a good reason, it's not worth the everyday effort and you have to build your sense of this around the everyday.
Non-obsessed cis women, in my experience, put in a standard amount of effort, something you'd expect of everyone with the added bit of more shaving, then makeup if they're going somewhere slightly out of the ordinary.
I assure you, "10+ years of practice" is only really 10+ years of practice in a very insecure person, who doesn't like how they look.
