It's a song about the abrupt and often unfair life changes that come from unprotected sex, unexpected pregnancy, and ill-prepared parenthood (especially motherhood), the deceptive nature of our sex drives that lead us together to make babies, and the weird social justifications and norms we have for glorifying reproduction for the sake of itself. It uses well-tread sex change operation memes as a crass and purposely repulsive metaphor because sex causes changes. Yeah it's that simple. It's a bit intellectually lazy and blunt, not to mention casually condescending to people who actually get sex changes, but crass and blunt is kind-of her aesthetic.

I identify more with Girl Anachronism, personally.
