I enjoy depictions of guro-tier gore and physical/emotional suffering in fictional works, since they often have an emotional effect on me. You need to distinguish just being overweight(more body fat that is considered "beautiful", but no health risks) and actual obesity(health risks, noticeable physical impairment).
The negative attitude towards people who are nowadays considered overweight is a purely social and relatively modern thing, while obesity is a legit medical issue.
