The LGBT movement, at least in the US, started in California. The gold rush meant that there were lots of men in California, but not a lot of women, since men were typically more fit for mining jobs. Due to the shortage of women, a culture of cross-dressers and drag queens developed, so that you could still go see strippers at a gentleman's club, but not feel gay about it. Of course, this meant that people would be more open about things. Some of these cross-dressers were gay men, some were trans women. Also, the women that DID go for mining jobs were typically really masculine, some being butch lesbians, and some actually being trans men.

Basically, in the 1950s and earlier regular straights just assumed that Gay = Taking it like a woman, therefore Dressing like a woman = Hypergay, and since they wrote all the documentation and controlled all the oppression (yes, back then there really was oppression, shit was illegal, it's not like now when "muh oppression" is the clear sign of a Tumblr mong), and there was nothing like treatment, gays and trannies got lumped in together. Notably also they lumped *themselves* together because they also belonged to the culture that believed all this, so they thought of themselves in those ways.

There's still a bunch of stuff left over from that era that's uncomfortable and contentious between the groups, notably drag and acting swish/flaming -- basically, various gay forms of acting feminine which arose due to that era's beliefs about homosexuality.

The Queer Rights Movement is an all-inclusive umbrella that promotes legislation for the protection of all people who are not explicitly heterosexual.

A trans woman threw the first brick at Stonewall. A pretty decent chunk of the founders of gay liberation were trans, or were funded by trans people. Much of the LGBT movement rests of the shoulders of the long silent and neglected T, and it's only recently that it's changed. If anything trans people were integral until gays and lesbians decided to throw the T under the bus when they shifted the movement to try to make straight people see how they were just cishets, and then they just wrote trans out of the gay history books or called us crossdressers and drag artists. Also a lot of the vocabulary just didn't exist back then either.
