Not all HSTS's transition at a very young age; actually, many probably lived as very feminine gay men long before transitioning (just like Blaire).

20 is the median age of hsts transition (half transition before, half after), or at least was in 2009 when nuttbrock (a noted opponent of blanchard's, natch) studied as many trans women in nyc as he could find and accidentally found they fit the typology
michelle hendley is also hsts and transitioned at that age

i can't find numbers on this one and that's also highly suspectible to change
as recently as 2014, the median age of *all* mtf transition in the uk was quoted as 42, and it can be safely assumed the majority of trans women in the uk have agp etiologies because it's a high-individualism western country
so 'mid-40s' is a roughly accurate estimate historically, but things have changed rapidly and in circles that attract a lot of agp trans/pre-trans women (e.g. ambiguously-autistic silicon valley programmers, 4chan) they routinely transition in their late teens or early twenties
