It pretty obvious now that Akko is supposed to be an early transitioner MTF teenager. Her behavioral traits are typical of straight, early transitioning transwomen who try to forcibly inject themselves into female acceptance through extreme histrionics and domineering and persevering extroverted expression. This behavior is also borne out of the extreme self advocation and self regulation that early transitioners have to go through when contextualizing themselves with their peers.

You can also tell by her proportions in that she has a huge cranial dome in contrast to her tiny jaw which is typical of MTFs who begin puberty blockers and HRT in the 10-13 age range.

With the nerdy looking one, the outline is likely much closer to what the head would be like as it doesn't go straight on the hair. It goes in properly as it would with anime art. As for the one Cara has HCed as a tranny, there's no space allowed for the hair to be off the skull. There's even a spot where the red outline goes above the hair.

If you were asking about how she's more developed, you can see that the blonde one is made to look younger by her stance, the glasses shape and size, the fact that her chest is less defined, and the fact that he clothing hangs off her body a bit more. The one with darker hair has a more feminine shape to her and her clothing is a bit more snug in areas that accentuate femininity (hips, ass, chest, upper arms), making her appear to be older.
