North America and Europe have patent laws and distribution laws regarding pharmaceuticals. It's designed to protect pharmaceutical companies from companies producing generic medications and flooding the market with cheap meds.
Only certain brand names are allowed to be prescribed in certain areas and the laws all protect against outside interference.
If estrogen was OTC then there would be no brand protection in place and the companies producing specific brand names would no longer have profit protection.

These laws exist because the concept is by ensuring profit protection it encourages original research. What they say is if we didn't guarantee a company that they would have zero competition for X amount of years they might just not research anything to begin with. It's pretty debatable if that is true but that's why we have what we have.

It's significantly harder to ensure. The way it is right now doctors and insurance companies are bound to specific brands.
If something was OTC they could bring it back and forth across borders without regulation, they could import it, and it would be a major hassle legally speaking to prevent generics from coming into the market.

Right now there's no real loose ends in the system other than illegal importations. This kind of medication and access to doctors who could prescribe and treat you are very hard to get in south america, your best bet would be buying things from the gray market online and even so you would be paying them in dollar or euros when your country currency is very weak (ex: if a box of cypro is 30 dollars they would be paying 120 of their country currency when the minimum wage is let's say 700 of their country currency).

Also there's the fact we don't really have a lot of doctors specialized in this kind of thing, our endocrinologists really have zero experience with transgender people with very few exceptions and those are very expensive and hard to come by.

I think what made everyone think south america is the land of MtF was porn but most of the people in there either don't even take hormones or when they do and have had some kind of plastic surgery they were only able to afford it due to working in the sex industry.
