OP here. I should clarify that I've only used AKs and short barreled carbines (aside from my primary Mac M11 and P90) so this is why I prefer this. On a Mk18 the magwell grip is fucking perfect for me. So damn comfortable.
And I'm rarely in a position where I'm rapidly transitioning between mutiple targets that are for some reason just standing there staring at me; a magwell grip is fantastic for leaning around corners (or my ballistic shield) and peaking.

You dummy. The C-clamp grip is supposed to help with transitions and follow-up shots in close quarters - you're not supposed to supposed to sustain it for long periods of time. Also, there's no reason for the thumb to wrap around your handguard. Just resting it on the top is fine. If you have a tapeswitch there for a light/laser, that's even better.

My thumb has limited movement and can't wrap around the top of the grip anyway. Hey OP, I got a recommendation for you. I too prefer magwell grip, however, after noticing it affecting my accuracy I abandoned it altogether. Despite this, I missed it as it was comfy for me. I soon found a compromise however.
Its simple really. Just get a foregrip, preferably a stubby one, but instead of actually gripping around it rest your thumb up on the fore guard of your rifle. You can have what is essentially a magwell grip but to the fore end of the weapon which helps accuracy and all that jazz. Still comfy (especially for someone with hand issues) but much more effective with standard and long size rifles.
I've heard people say you shouldn't use foregrips unless your weapon is full auto, but hey, it works for me. Just a suggestion.

The c-clamp grip is gay and it really only works on AR's with full length handguards. On a lot of other guns you're either grabbing hot barrel and/or covering your sights.

I either go with a traditional rifle grip on the handguard or sometimes I use a relatively short vertical foregrip. If my AR doesn't have a lot of shit hanging on it, I just hold it like a rifle.

I have one carbine set-up with short handguards and a quadrail so it makes the front end kind of heavy, so I put a VFG right at my last rail spot and it's perfect. I have my flashlight out as far as it can go as well, so I can manipulate the flashlight with that hand. There's still enough space that I can hold the rifle normally if I want too.

As for elbow position, chicken winging is a good way to expose your arm to catastrophic damage.

I've taken some carbine courses and they all teach a traditional elbows-in small-target stance

While it may seem counterintuitive, that's not actually a positive. Competitive shooting is by definition a precise environment. It's all about every edge you can get in those few minutes of shooting. The "quicker transition between targets" is a real thing I suppose, but it's pretty negligable under real world circumstances. You aren't going to be doing that shit for 2 hours of room clearing, it just doesn't make sense
