Because Russia built submarines, learnt lessons and improved the next design.
Whereas the west spends far more time testing and developing to try and get right.
Also russia had far more submarines at sea than NATO so they are more likely to have something go wrong.

I actually heard from multiple RN and USN submariners than when a new Soviet SSN or SSBN was built and on their maiden voyage, they'd have serious problems keeping track of them with sonar and noise.
It was only due to the poor maintenance quality after roughly three years did it make it easy for NATO to keep track of Soviet subs,
Russia built more subs, with a much wider variety of designs and more extreme mechanical stresses.

Not at all, they also maintained them poorly and crewed them with conscripts. But given how shitty the conditions were it's amazing to be how few accidents the Russians had on their boats.

Western sub designs were more mechanically conservative, with greater emphasis on sensors and quieting. With the benefit of hindsight we can say this is a better approach, and safer.
