Incident was caused by lackluster regulation due to flight op's, falsely advertised fail safes (Comp B rated for 10 minute fail safe under fire, they detonated under 2 minutes) and then lack of DC training for rest of the crew once the primary DC teams were killed.

It's why damage control is indoctrinated on every at boot so everyone knows their role. Forrestal and the Sheffield are primary reasons why USN DC capability is so stupidly good.
