The first number indicates the number of rounds in the magazine.

The "+1" indicates the ability to also chamber a round, in addition to the magazine capacity.

The typical way to do this is put any number of rounds in the magazine. Insert the magazine, then rack the slide to get a round in the chamber. You then release the mag, fill up up to capacity, and put it back in the gun.

Only do this if you have proper instruction and experience.
