Regarding legal issues in the United States:
Is carrying a gun within a gun case considered open carry, concealed carry, or neither?
Does it matter if the case is locked, or if the gun is loaded?
What about carrying a gun in a backpack?
If I am in a state that requires a permit for both concealed carry and open carry, and I do not have a permit, can I still carry a gun within a case or pack?

In texas having a gun in a case even if its a case that say "glock" is considered concealed, doesnt matter if locked or loaded, same with in backpack. Depends on teh state but no u cant carry it unless ur bringing it to ur car or something like that

What if I live in a city apartment and my car is parked on a street several blocks away due to difficulty finding parking? Can I then carry a gun in a case without a permit between my car and home, in a state where permits are required?

Other than in your driveway or your local range's parking lot, why would you be carrying around a gun in a locked case at all?
just wanna say I've flown with weapons in cases and they never hassled me about CC paperwork
if they're locked and unloaded they don't give a shit
Jesus, fucking cuckstates get your shit together.

as everyone else has stated it depends on the state. generally though if the weapons is in your possession (as in you can quickly get to it) you're carrying one either CC or OC
Carrying in a backpack is usually considered CC'ing.
If the weapon is locked in a case you may be carrying. Is it in your car trunk? not carrying walking down the street? they might consider that a CC.
You really need to look up your local laws.

Texas here. No lawyer, but from my behavior i would say its legal to carry from your home to your vehicle without a LTC. Duh

California requires all guns to be transported in locked cases.

The only state I can think of with provision for the scenario is Arkansas. In Arkansas if you're over 18 traveling outside the county you live in you are consider to be on a journey and you may carry weapons (even a pistol, even if you're not 21) but you cant CC them. and again this applies only to traveling outside your home county but still in the state.

For transport, either when traveling without the use of a vehicle (by foot or bike, for example) or when parked a ways away from home and walking between home/car.
I live somewhere where I don't have to worry about this, so I'm not asking about a specific state. But any state where it's required to have a permit to carry. What if someone doesn't have a car and commutes by bike? How can they legally transport a gun from place of purchase to home?
