What does "dealer" mean in the context of this page? https://www.nraila.org/gun-laws/state-gun-laws
Sometimes it says "licensed dealer" but in some cases it just says "dealer." When it just says "dealer", does it refer to anyone selling a gun even in a private sale, or only FFL dealers?

Take Washington (state) for example. A "dealer" is not allowed to sell a pistol unless the purchaser has a license or undergoes a background check.

Another state for which it uses vague wording is Minnesota. A handgun or "assault weapon" can be sold by a "dealer" only if the purchaser has a permit or has filed a transfer report.

In these situations, do these requirements apply to private sales, or only FFL sales?

It depends on the state. In washington you must find a FFL to facilitate the transfer. In minnesota there is no such requirement but since there you can be held responsible if someone who was ineligible to purchase your gun buys it and uses it to commit a felony within a year of the purchase, some prefer to pay to get a FFL to do it anyway.

And in Minnesota that article says permit or transfer report is required for assault weapons and handguns. Is that only for FFL, so private sales of assault weapons and handguns don't require anything like that?

Yes, private sales without going getting a background check through an FFL are illegal in WA. Not only sales but any transfer of ownership, which here includes lending, must go through an FFL. (Do not comply)
