Poor people without insurance who end up having to go to the hospital, usually don't end up paying their bill, or they get the fees almost totally waived. This drives up the cost in general.

Not only does the average Brit have more comprehensive healthcare than the vast majority of Americans, but when you factor in the elected cost in the US to the taxed cost in the UK, the price difference is small. Also, imagine not having to worry about deductibles.

The only thing we have them beat healthcare wise, is we spend less time on average waiting on an appointment than brits, because they schedule their appointments based on apparent urgency of the reason the patient is scheduling...whereas we are customers, and there's the business incentive to get to people very quickly.

The US is an empire in denial. The guys who run the show know its an empire, but they have an entire media-information-entertainment apparatus that's devoted to keeping the public laboring under the assumption that isn't. You don't have a normal old country that isn't engaged in some sort of dodgy business with one hundred-something military bases in foreign lands which, along with embassies and other US-owned garbage, are used for "intelligence" missions abroad.

Unlike Britain did from 1350ish (when they began holding parts of Ireland long-term) to 1960ish, the US runs an odd arrangement whose closest historical parallel I can think of is Carthage. We have oodles of client states, treaties, mercenaries, etc. Obviously the part where this analogy falls apart a bit is that we do have a professional army as well, which Carthage didn't really do as much; but nonetheless, at least for imperial management, I think it holds well.

Brits after WWII decided to throw away empire and take on the welfare state. Both are expensive, and as far as the state's budget goes, it can sustain only one and not the other really. The US is trying to play a bit of both and ungodly debt is the result. As for why Brits made this choice, I think it was a result of WWI and WWII - the costs in life and treasure. For one, Britain was debt-laden and pouring money into India, Africa, and the Carribean didn't make much sense. Britain had a choice of war with Germany or keeping empire, and choices were made. For another, a lot of people figured, after all this war, the Blitz, etc., might as well have cushy pension and healthcare. Too bad you had cunts like Atlee too.

I should note too that universal healthcare is associated (rightly) with social democrats - that is, Fabians, gradualist socialists, etc. Guys like the Labour party in Britain. There's always been a healthy distrust of social democrats in the US - there never has been an outright Fabian or social democrat party, though they did make the Democrat party basically the de facto party of communists, minorities, and the "elite" if you like.

This has meant that gun rights haven't gotten fucked too hard yet nationwide, but we still have a number of social democratic reforms. We have a national pension, we have national healthcare for the infirm and elderly, disability insurance, healthcare for military veterans, and some other shit. All of this dates to either the New Deal (around 1933) or LBJ's Great Society (1964/5). All of it is ridiculously expensive. Healthcare spending in general is out of control, in part because waste and corruption are rife, but also because you have shitloads of people trying to eek out extra weeks or months in the last years of life. Obviously, for people who cling to life, especially for ones on medicare, end of life care and ostensibly life extension has a perfectly inelastic demand curve.
