SAMs vary in size massively. The USSR designed several large SAMs with long range and high altitude capability. Example dimensions:

Not really, Israel has regularly struck inside Syria at things it regards as a threat, and it's an open secret that Syria supplies weapons to groups fighting Israel such as Hezbollah (both on its own volition, and as an Iranian proxy).

It's a game both sides play, with both sides knowing roughly what is and isn't a tolerable escalation.
