Like the tail gun that was removed from B-52s, they made sense when interceptors were likely to go for gun kills, back when the aircraft were designed. They don't make so much sense now.

The Russians have chaff and flare cartridges for their 23s. I'd guess that they keep the guns on the planes mostly for those and not for use as normal guns.

There's special 23mm ammo that burns extremely hot, magnesium casing.

The tail guns on the Il-76 carry very little ammo, and in dangerous situations, they carry almost exclusively "hot" flare ammo.

Doctrine is that if the gunner sees a MANPAD launch, he shoots at the incoming missile, and hopes it fools the missile into chasing it and not the plane.

Missiles are small, fast, and very difficult to hit.

Also, if the missile explodes too close to the plane it can still get a proximity kill or damage from the shrapnel and concussion wave.

Much better to just not let the missile hit you in the first place with EW and countermeasures.
