How exactly do submarine captains receive their orders? Do they need to surface to receive instructions or is there some sort of underwater communication system in place?

They get comms when they surface or raise a comms buoy. The could be told to surface using extremely low frequency radio waves, but those take too long to do anything but tell the sub to surface for further messages.

Just way to expensive to continue operating (hundreds of miles of antenna wire consuming several hundred megawatts of electrical power). Not quite, the two US stations had lines that were 14-28 miles long, and took a few megawatts to run. But ELF systems are just bonkers in general. The US transmitted at 76 Hz, which has a wavelength around 4,000 km if i remembered right. Its also extremely low bandwidth, like 1 bit a minute.

Or the launch buoys that can transmit recorded massages. Recent version of buoys can work as re-translator nodes communicating with submarine via acoustic channels and with satellites via radio channel.
