I'm a bit new to prepping for when SHTF so I'd like to know if I should buy a premise medical kit, or build my own? Cheapness doesn't matter so much as functionality.
I live near a temperate cave system and ravines so I need something that I can patch myself up if I break my leg from a fall, and also in case I get shot or something.

You can buy a military surplus or camping one for pretty cheap that are pretty good. If you wanna build your own, minimum you want is:
Bandages/gauze wraps ranging from small to large
Water purification tablets
Painkillers (aspirin, paracetamol, ibuprofen etc)
Penicillin based antibiotic/iodine
Small pair of scissors
Plasters
Cleaning wipes
If you know what youre doing then also:
Razor blade or scalpel blade
Emergency traichiotomy tube
Tourniquet
1-2 foot length of surgical plastic tubing

if you need an emergency tracheotomy tube when SHTF, then chances are whoever needs it is going to die in the long term

True, but in a SHTF scenario diahorrea could kill you in a few days without clean water and meds anyway, so long term is more about skills and resourcefulness than gear id assume

If you want a good first aid kit, either go will a roll up and fill it wit a lot of gauze, super glue, and neosporin, stool hardeners and softeners.
That will last you everything but a break. For breaks you want a sam splint and triangular bandages. At this point you need an assault pack/24 hour pack.
