What would cause this on only the last round in an AK pattern

Probably a magazine issue, could be the spring or maybe the it pokes out of the mag after the second to last round is pushed out.

If it only happens on the last round, I would assume it's a problem with the magazine. Spring is probably worn or dirty magazine

Buy some better mags or at least swap out the springs in your mags for extra strength springs.

Surplus galil mags are shit if that's what you have, certain ones I have will occasionally push the round down and chamber nothing because it has a weak spring, I haven't tried them yet but tapco makes galil mags

You can rebuild them easily with Magpul followers and 40rd AR magazine springs.

Wolf Springs also makes an "extra power" AR spring that will work.
