No, you didn't. When someone says to inspect the chamber, you should have the action open or the slide back and be looking both into the chamber and down the magazine well. If you inspected the chamber, you would have seen the magazine inserted with cartridges loaded, realized the weapon was loaded, and have avoided an ND.

From what you are describing, you did one of two things: you dropped the magazine, looked into the chamber, then someone put the loaded magazine back in and you released the slide without giving it a second thought, or you failed to look into both the chamber and down the magazine well at all. Both are incorrect techniques. Your bad technique was the cause.

It broke her bone. There doesn't seem to be any nerve damage, but she's still very numb and swollen so it's hard to tell. It tore the ligament to her thumb but the doctors say they fixed it and she should be 100%. The bullet passed through her arm and landed in her thigh. The thigh wound was slight. I thought the whole point of an indicator was so you could *touch* check the weapon? As in, it's pitch black and you draw/grab a firearm- run your hand over the top of the slide and feel the bump: you're ready to go without having to press check / chamber check.
