It's a lot harder to get a deadly strain of a virus that you could also control and release in a certain area and guarantee deaths, in most first world nations more than likely there would be a cure. Also it's less shocking and less instant, imagine if isis went "AHAHAH INFIDELS WE INFECTED ALL OF WASHINGTON"

It might cause panic, but more than likely it could be contained and handle. It isn't shocking.

Compare that to running over masses of people during a festival or planning controlled attacks, it's a much higher shock value and a lot easier to coordinate and execute.

The only thing they could really do thats *similar* to a deadly virus would be a dirty bomb, but even then they would have better luck with controlled attacks with guns.

tl;dr its easier to get guns and ammo and tell muhammo over there to go shoot at a crowd rather than to try and coordinate a biological attack

I imagine manufacturing enough to make a major impact is pretty hard to do on your own. If you try and do it with others, that's when you get caught because you communicate with other people and security services can monitor you. It very hard to predict and stop a lone wolf attacks. That's why IS does it. It has a decent impact and is hard to stop. You also don't need support from higher ups in the group.

Even first world countries don't know how fight bacterial infections that have developed drug resistance to all of our antibiotics. It wouldn't be hard to create drug resistant strains of a bacteria. You just need to keep exposing it to antibiotics and it will do the work for you.
