an adapter is not strictly necessary for use in america, but without it it will always be a bit hotter than "normal" at any given setting, which may lead to a fuse blowing in it or damage to the table surface if run on high.

As for myself, I would never buy a kotatsu. I don't like to sit on the floor, it's really uncomfortable. I have no idea how the nips do it, is it because they have short legs or something?

Brought a fairly high end rice cooker back from my time in Japan, it works fine. My concern was the difference in frequency rather than the slight voltage difference, but the electronics have worked fine for over a decade.

I'd say a simple resistance element heater with a bimetallic switch would be fine.
