She won't live forever but her lifespan is significantly lengthened from huffing Remilia's dust which is why we hardly see her aging despite all the years that have passed.

Almost every team's extra talks about her immortality and mentions you could become immortal by eating her liver. Reimu, Marisa, and Sakuya all turn down the opportunity because they'd rather remain human. The youkai don't need it. As for the ghosts, Yuyuko tells Youmu that eating her would pass the Hourai curse on too, and warns her not to do it.
