The life counter is the red cubes on the bottom left, the blue ones are barriers. The actual life and barrier percentage is to the right of it.

Took a while for me to like this, but now it's one one of my favorites.

Should I stay level 0 or should I upgrade? I want a good challenge but I don't want to go up against something impossible when I go for an unreal 1cc. Also how do extra lives work? In one playthrough I got an extra life from almost every boss but today I didn't get a single one by stage 6.

Up to you really. The game is really unforgiving on Unreal and you'll cry even as a Touhou veteran. I'd at least get Hooaka's upgrades to mitigate the power loss per hit. Extra lives drop on 3rd and 5th stage bosses, aswell as extend events but you have to clear the cell without bombing or getting hit.

Outside the first one, the Len'en games are pretty solid imo.

I love the danmaku patterns and gimmicks, I would dare to say that the music has a punch factor that recent touhou games are (partially) lacking.

The music part is probably due to the instruments. ZUN uses VST's now more than ever, with very few exceptions, while JynX uses hardware alone, specifically the SD-20 and the SC-88pro. In this last game, JynX ended up using a lot of instruments he wouldn't use normally and this is why this game's OST is very good.
