ZUN can't really draw breasts obviously, and characters generally wear loose dresses and things. If you look for them you can see them though, even if they're subtle. Plenty of characters ARE flat though.

Yuyuko is a bad example since she is supposed to be the ghost of a young girl despite the hag meme.

Sanae has always been drawn with relatively large breasts, so he can draw breasts if he wants. Thus the majority of Touhous, which btw are usually teens or young girls for some reason or other, are generally flat.

Probably. Immortality or long life is just an excuse to have characters be young girls forever in lots of other Touhou cases.

Although there are Touhous who have had kids and so have to be adult. Also Byakuren was an old hag who made herself young again.
