Probably not, the smallpox epidemic hit right during the siege of Tenochtitlán, right after the events of noche triste. Change the sequence of events for that and you screw the whole timeline.
No conquest of Mexico means no agricultural infrastructure in the new world, no standing armies waiting to take on Spanish orders, no lucrative source of wealth to pay for costs and motivate further conquests. Peoples like the post collapse Maya or the Huastec, Zapotec or Tarascan (all as technologically advanced as the Aztecs) don't fal in quick sucession, the Philipines don't get conquered, etc.
Had the Aztec survived its first encounter with Europeans it might have sought alliances with other nations, it could have ended up trading with the Portuguese, the Dutch or the French and Mesoamerica with steel, gunpowder and horses would have certainly changed the whole power balance in the region, you may not have gotten Pizarro's conquest of the Inca, nevermind settlers in North America wouldn't have found the Indians weakened by disease. There's no telling how the history of the Americas would have unfolded even if you assume all these people would have fallen to Europeans eventually.
