What caused the London fire? I've been trying to find news from mainstream sources (BBC, NBC, CNN, The Guardian, The Sun, The Independent) that explain why it happened but to no avail.

the way the insulation was done in the building made it a bonfire. fire alarm didnt trigger so the fire qickly spread to every floor thanks to shitty building plans

The outside of a building shouldn't be able to burn, it defeats all the interior design which is built around containing fires within rooms and floors long enough for people to get out.
Who approved this shit at government level? All this stuff has to be signed off on.
The materials, their installation, and fire suppression is the number one priority of any inspection especially in a high rise.
