Vozrozhdeniya Island between Uzbekistan and Kazakhstan has some insane levels of bio weapons and shit running around there, including once they let loose an haemorrhagic smallpox disease

WWI had the edge on civilian casualties/atrocities, but most of the actual battles weren't that big except in Russia. WWI battles were ginormous and the nature of the combat absolutely nightmarish.
