In other medical systems, particularly Britain/Europe, all of those fall under the major ones you need to initially specialize in: Surgery, Internal Medicine, Paediatrics and ObGYN

After you get those down you 'sub-specialize' so you don't fail to diagnose other specialty diseases like a retard
