you could use bitcoin but whether or not you can convince clients to deal with that extra layer of inconvenience is questionable. you could also accept payment with credit to an anon account on an intermediary service like amazon, the obvious limitation being that you can only use the credit on said service. i know certain artists who mainly do sfw but do nsfw personal comissions on the side simply restrict the rights so that the nsfw stuff is for personal use only.

You just create a reward with a certain amount of money (like $150 for a full rendered sketch) then ask to your commissioners to pledge for that reward. Then just wait to the beginning of the month to patreon to process the payment to your paypal account.

The advantage of this is that patreon have good terms with paypal about nsfw stuff (but i'm not totally sure if you can resolve a dispute in your favor), and the receipt of the commissioner will show the transaction to "patreon" instead of your name. The disavantages is that you cannot know the name of the commissioner neither their personal info. But who cares. I only recommend to work with people you can trust.
